Short Stay Intensive Rehabilitation - Enabling Independent Living

The service run by Uplands Care Centre has been committed to providing high quality nursing care for over 30 years and successful rehabilitation outcomes for individuals with an Acquired Brain Injury for over five years. We specialise in rehabilitation and re-enablement services, particularly for individuals with an Acquired Brain Injury requiring slow stream neurological rehabilitation.

The centre provides comprehensive assessment, rehabilitation and therapy services for male and female adults and throughout their stay with us, our patients are encouraged to make individual choices about all aspects of life at the centre and to be as independent as possible. The centre employs a physiotherapist, occupational health therapist, speech and language therapist and psychotherapist.

Uplands Rehabilitation Centre in Westcliff-on-Sea is an innovative and unique 17 bed service providing short stay therapy led rehabilitation and re-enablement services to people following events such as Strokes, Accidents, Acquired Brain Injuries or Orthopaedic problems.

Akash Soni

Director and Nominated Individual

Akash is having 20+ years of experience in owning and managing Care Homes. Akash has been instrumental in overseeing the environmental and operational changes to Uplands, manages the relationship with commissioners and provides Regulatory support.

Bob Mutubuki

Registered Manager

Bob Mutubuki is the Registered Manager and oversees the daily operation of the service, adherence to all Policies and Procedures and implements the Quality Assurance systems.

Kerry Soka

Deputy Manager

Mrs Kerry Soka is the Deputy Manager / Clinical Lead with primary responsibility for ensuring safe admissions into and discharges from Uplands. Kerry personally undertakes all pre-admission assessments and remains each patient’s primary point of contact throughout the duration of their placement – co-ordinating their care and therapy interventions. Kerry oversees safe discharge planning with the assistance of the resident Occupational Therapist and dedicated Social Worker.

Each of our residents has a rehabilitation programme, which is formally reviewed on a weekly basis and where appropriate, decisions are made using the metrics derived from tools such as the Barthel Scale and GAS Scoring amonst others.

Uplands will generally only provide a short term placement for people who have an expected functional rehabilitation outcome.
